PURPOSE OF POSITION
The Adult Complex Care Case Manager provides a specialized case management function to implement a Ministry of Children and Community Social Services funded service plan for adults who reside in a supported living environment on a fee for services basis, or who reside in their own or their family home and purchase the services they require to meet their needs. The Adult Complex Care Case Manager provides ongoing oversight and evaluation to ensure that the plan continues to be relevant, and in the individual’s best interest. The Adult Complex Care Case Manager ensures that the funds are being utilized appropriately and according to their service plan. The Adult Complex Care Case Manager also provides support and referrals to access a range of required services.

PURPOSE OF POSITION
This is a specialized case management function that provides, families and children/youth with Autism presenting with an urgent response with:
To provide coordination of Urgent Response Services in a comprehensive, flexible and timely manner. This will include completion of the screening tool for eligible children and youth, leading the development, monitoring and evaluation of the 12-week Urgent Response Service plan.
The Urgent Response process will be inclusive, accessible, and culturally appropriate. It will be respectful of the values and meet the diverse needs of children, youth and their families.
STATEMENT OF MAJOR RESPONSIBILITIES
1. To determine eligibility by completing the province wide secondary screening tool to determine
eligibility.
2. Developing a strengths-based Urgent Response Service Plan that addresses the service need of the child/youth, which is driven by the presenting urgent need.
3. To participate in professional development in consultation with manager, and to initiative in learning and developing new techniques of service delivery.
4. To participate in community and agency committees as required.
5. To adhere to all agency standards, policies, and procedures.
6. Must be aware of and adhere to Health and Safety Policy, Rules and Regulations.
7. An organized and accurate electronic clinical record will be maintained that demonstrates the provision of services, establishes accountability, and enable the evaluation of service quality for all persons served.
8. To carry out other duties as assigned.

This position will lead our Human Resources Department (HR), providing guidance on all aspects of human resources, including supervision of the HR Manager This position is responsible for developing and implementing HR strategies that align with our organization mission, vision, and values and the creation of equitable, diverse, accessible and inclusive workplace. This is a key leadership role that works closely with the Executive Director and other Senior Leaders to foster a positive and inclusive work environment.
Strategic HR Leadership: Develop and implement comprehensive HR strategies that align with the organization’s strategic directions and overall goals and objectives.
Recruitment and Talent Management: Oversee the recruitment process, ensuring that all hiring practices are fair, equitable and compliant with labour laws, policies, regulations and agreements. Oversee the development of enhanced recruitment processes including job postings, advertising, interview team skills and participation selection panels as required.
Employee Relations: Foster a positive employee relation program by promoting a culture of respect, inclusivity, and open communication. Participate and playa leadership role in Management and Union Joint meetings. Develop a day-to-day working relationship with union members and staff sharing information on a timely basis and meeting to discuss issues as they arise. Work with Management at all levels providing advice and training related to human resources policy and administration. Provide expertise in the development of employee relations activities that promote progressive programs focused on employee development and succession planning.
Benefits and Compensation: Develop and implement competitive compensation and benefits packages that attract and retain top talent. Participate in meetings with outside representatives, review and discuss cost of health benefits, overseeing the management of the Benefit Plan (Pension, Health & Dental, Long Term Insurance Plan). Direct and oversee the classification and salary program including pay equity, ensuring position descriptions, organization charts, salary grids and related documents are accurate and recorded appropriately. Meeting and advising management staff regarding financial and classification issues that may be impacted by organization changes.
Performance Management: Implement performance management systems that support employee growth and development and the development of enhanced record keeping processes. Providing advice and guidance to Managers implementing Staff Performance Programs.
Employee Engagement: Develop and implement initiatives that promote employee engagement, retention and satisfaction by participating on Committees i.e. Employee Health & Safety, Wellness, and other ad hoc groups that are developed.
Labour Relations: Negotiate and administer Collective Agreements and manage labour relations matters in a fair and respectful manner. Oversee all labour activities that arise and assist managers with disciplinary issues and ensure appropriate documentation is maintained. Participate in all HR practices and policies are compliant with relevant laws, regulations, and standards. Participating in leadership meetings and interfacing with other departments on a regular basis, advising as necessary on labour laws & or policies that may be relevant.
Training and Development: Interface with program leadership to develop and implement training programs that support employee growth and development. Advise on and recommend training programs that support the goal of achieving an equitable, diverse, accessible and inclusive workplace.
HR Administration: Oversee the administration of all HR functions, including employee information systems, benefits administration, salary and classification records and functions affecting payroll. Oversee all aspects of leave of absence in accordance with agency policy and legislative frameworks.
Health & Safety Program: Direct and oversee a joint union and management health & safety program and an agency wide health and safety program. Ensure all policies and regulations are adhered to, and regular inspections are completed and posting information as required. Chair a monthly health and safety meeting as per legislative requirements. Meeting with local health and safety inspection officials regularly, discussing and resolving matters needing attention and reporting to the Executive Director.
· Bachelor’s degree in Human Resources, Business Administration or related field.
· Minimum of 5 years Human Resources experience with two to three of these in a senior role preferably in a social service organization.
· Experience working in a unionized setting.
· Excellent communication, interpersonal and leadership skills.
· Strongknowledge of employment law, labour relations and HR best practices.
· Skills in data analysis and the ability to address challenges through a strategic lens.
· Strong technology skills and the ability to use technology to promote strategic development.
· Solution-focused development strategies.
· Demonstrate organizational skills and the ability to multi-task.

Salary Range: $81,258 to $91,787, plus a generous Employee Benefit Package, including a Pension & Health and Dental Plan.
