Intensive Service Coordination provides information, consultation, and community case conferencing for Simcoe County children and youth up to nineteen years of age, and their families. This service is designed for individuals who require extraordinary support from health, social, and/or educational services because of severe emotional, intellectual, developmental, and/or physical needs.

Children and youth supported by Intensive Service Coordination have needs that are beyond the capacity of the local system, and all possible community options have been previously exhausted by families and service providers.

Support Provided Through the Program:

Provides information to help families and service providers make informed decisions
Works closely with the community Case Manager to encourage linkages with all available community resources
Acts as a neutral third party during case conferences
Aids in the development of an Integrated Service Plan for the child, including inter-agency service provisions and roles. This could include accessing out-of-area resources if needed and available

This service provides support to children and youth whose needs are beyond the capacity of the local system. All possible community options must have been previously exhausted by families and service providers.

Funding is provided by the Ministry of Children, Community, and Social Services. There is no fee for this service.

Service providers (with consent) and in some cases, families may directly request a referral if the child or youth has two or more diagnosed special needs requiring coordination of services from different support sectors. The child or youth’s team must agree with the referral.

Simcoe County Community Action Program for Children (CAPC) and MotherCare (Canada Prenatal Nutrition Program – CPNP) are a part of a national program with then 25 years of experience supporting families in this region. We support pregnant participants, young parents, and Newcomers to Canada with children 0-6 years old who are currently living on a lower income.

Data Collection Consent and Required Information

We do not maintain participant “notes” related to your participation with us but we are required by our funders to track attendance and the resources we distribute. We use an electronic data management system to record and store minimal personal information and program attendance. This information includes: name, date of birth, income, due dates and/or dates of birth of your children. Other information that we request is optional for you to provide.
